The below information is a guide to the information you will need when filling out the Minnesota Business Tax Identification form. You are required to fill out this tax identification (PDF) form, providing the information requested and acknowledging your understanding and acceptance of the information below.
Please note: This is not the only form you will need to fill out to receive a food license. Please refer to the applying for a food license page for information and procedures on obtaining the other forms you will need when applying for a food license.
Minnesota Business Tax Identification Form
Pursuant to Minnesota Statute 270.72 (Tax Clearance; Issuance of Licenses), Subdivision ,4 the licensing authority is required to provide upon request of the Minnesota Commissioner of Revenue your Minnesota business tax identification number and the social security number, name address and business name and address (Federal Tax number if no State number).
Under the Minnesota Government Data Practices Act and the Federal Privacy Act of 1974, we are required to advise you of the following regarding the use of this information:
- This information may be used to deny the issuance or renewal of your license in the event you owe Minnesota sales, employer’s withholding, or motor vehicle excise taxes.
- Upon receiving this information, the licensing authority will supply it only to the Minnesota Department of Revenue. However, under the Federal Exchange of Information Agreement, the Department of Revenue may supply this information to the Internal Revenue Service.
- Failure to supply this information may jeopardize or delay the processing of your license issuance or renewal application.
On the application, you will be required to provide the following information.
- Applicant’s name (last, first, middle initial)
- Social Security Number *
- Home address
- City, State, Zip code
- Phone number
- Business name
- Type of license applied for
- Business address
- City, State, Zip code
- Phone number
- Minnesota Tax Identification Number (or explain why you don’t have one)
- Federal Tax Identification Number
* If company stock is publicly exchanged, you may omit submitting this Social Security information.
