Written Verification of Corrections
Please make note of the following information and procedures regarding the written verification of corrections form. Below is a guide to the information you will need when filling out the written verification of corrections (PDF) form.
Food establishment written verification of corrections
Risk Factors (RF) and Public Health Interventions (PHI) must be corrected immediately; Good Retail Practices (GRP) must be corrected within 48 hours. An action plan must be submitted for each item with corrective actions within 48 hours.

Information on form
On the form, you will need to provide the following information:
- Name of establishment
- Inspection date
- Signature of person in charge
In addition, for each corrective item, you must provide the following information.
- RF/PHI number or GRP number
- Date of compliance
- Corrective action taken
